Medical Health Statement
All students, whether traveling internationally or domestically, need to have their medical health statement form filled out. Please download the form under "Learning Content" on your CCE profile. Once you have had a medical professional fill out the information, scan and upload the document under "Application Questionnaires" on your CCE profile.
Carroll University Student Health Services 2018/2019 School Year
If you are interested in having your medical health statement completed on campus at the health center, domestic CCEs are $15 for the visit and International CCEs are $20. These visits are not physical exams, they are more of a review of the students’ medical history and what might affect their participation.
For domestic CCEs and non-exotic CCEs (i.e. Europe, UK, Ireland, Australia, etc.) students can come during walk-in hours and do not need an appointment. However for CCEs to exotic locales (i.e. India, Bali, Costa Rica, Guatemala, Africa, etc.) students do need to schedule an appointment to review recommended vaccines and prescriptions.
Regular academic year hours:
Monday-Thursday 9:00 A.M.-1:00 P.M. & 2:00 P.M.-5:00 P.M.*
Friday 9:00 A.M.-1:00 P.M. & 1:30 P.M.-4:30 P.M.
*Wednesday afternoons are reserved for physical exams, pap smears, and CCE Medical Health Statement visits to exotic locations by appointment only.
