Medical Health Statements

All students, whether traveling domestically or internationally, need to have their medical health statement form filled out and uploaded to their application after acceptance.  Medical health statements must be completed by a medical professional (Physician/NP/PA/On-Campus Health Center).  You cannot use your sports physical as a medical health statement.  Please download the form Medical Health Statement.pdf.  Once you have had a medical professional fill out the information, scan and upload the document under "Medical Health Statement" on your CCE application. 
Carroll University Health Center 
Health CenterVisits to the Carroll Health Center to complete the medical health statement are not physical exams, they are a review of the students’ medical history and discussion of what might affect their participation in a CCE.  The Carroll University Health Center has the medical health statement form online, so there is no need to bring a copy with you. 
Domestic CCEs 
  • $15 for the visit 
  • Students can come in during walk-in hours and do not need an appointment 

International CCEs 
  • $20 for the visit
  • Appointments 
    • Non-exotic CCEs (i.e. Europe, UK, Ireland, Australia, etc.) students can come in during walk-in hours and do not need an appointment 
    • Exotic CCEs (i.e. India, Bali, Costa Rica, Guatemala, Africa, etc.) students do need to schedule an appointment to review recommended vaccines and prescriptions

Monday-Thursday 9:00 A.M.-1:00 P.M. & 2:00 P.M.-5:00 P.M.* 
Friday 9:00 A.M.-1:00 P.M. & 1:30 P.M.-4:30 P.M. 
*Wednesday afternoons are reserved for physical exams, pap smears, and CCE Medical Health Statement visits to exotic locations by appointment only.
